CREDIT CARD ON FILE

To simplify billing, we ask all patients to keep a card on file, with the exception of those covered by Medicaid. Keeping a card on file offers several benefits:

  1. Streamlined Billing: It simplifies the payment process, allowing us to handle billing efficiently without needing additional follow-ups or paperwork.
  2. Faster Checkout: With a card on file, there’s no need to spend extra time on billing at each visit, making your appointment experience faster and more convenient.
  3. Transparent Payments: You can rest assured that any balance, copayment, or coinsurance will be processed securely and promptly, reducing the chance of unexpected bills later.
  4. No Interruptions in Care: By keeping payments up to date, having a card on file ensures there are no delays in accessing follow-up services or ongoing treatments.

Our Insurance Verification Worksheet is just one of the ways we’re here to help you feel confident and informed about your coverage. COMMERCIAL (GROUP or PRIVATE) INSURANCE PLANS

Our providers are in-network with the following commercial insurance providers:

  • Aetna
  • Regence BCBS
  • Care Oregon
  • First Choice
  • HMA
  • Moda
  • Pacific Source
  • Providence

There is a wide variety of coverage for naturopathic physicians.  This varies by company and by plan and it is important that you understand your benefits.  Some insurance plans obtained through the health market place require that your primary care physician be at an assigned facility, which will affect your coverage.

BEFORE YOUR APPOINTMENT

It’s essential that you confirm your insurance coverage directly with your insurance company. This ensures that there are no surprises or complications when it comes to billing and payment for your visit. Here’s what you’ll need to do:

1. Contact your insurance company: Give them a call to confirm that you can be seen at our clinic and that the services we provide are covered under your plan.

2. Specify the provider you’ll be seeing: When you speak with your insurance representative, make sure to mention both the provider you’ll be seeing at our clinic and their supervising provider (Dr Maeghan Cook). You may also want to provide them with the National Provider Identifier (NPI) of both your provider and their supervisor (Dr Maeghan Cook). The NPI is a unique identifier assigned to healthcare providers, and it helps the insurance company accurately verify coverage.

  • Meaghan Cook, ND | NPI: 156-889-9508
  • Colleen Amann-Shah, ND | NPI: 154-8527-724
  • Jennifer Samson, ND | NPI: 163-973-3801
  • Mary Fu, ND | NPI: 1487129524

3. Confirm coverage: Ask your insurance company whether your chosen provider and/or their supervisor (Dr Maeghan Cook) are covered under your plan. This will help you understand any potential out-of-pocket expenses you may incur.

By taking these steps, you’ll ensure a smoother experience during your visit to our clinic, and you’ll have peace of mind knowing that your insurance coverage is confirmed in advance.

If you have any questions or need assistance with this process, consider downloading our Insurance Verification Worksheet linked above. And please don’t hesitate to reach out to our clinic staff! We’re here to help you every step of the way: (503) 855-4341.

We accept the following OHP (Medicaid) plans without a referral:

  • Open Card (OHP Plus)
  • Care Oregon
  • Columbia Pacific CCO
  • PacificSource Community Solutions

Unfortunately, we are unable to accept the following plans:

  • OHP Trillium
  • OHP Kaiser
  • OHP Providence
  • OHP OHSU
  • All other CCO plans (e.g., Yamhill CCO)

    MEDICARE

Medicare offered through the federal government does not cover naturopathic physicians. This includes additional plans offered through Medicare itself.  However, there are Medicare Advantage plans offered through private insurance companies that do offer naturopathic coverage.  To determine your coverage, please contact your insurance coverage.

    UNINSURED

For those who do not have insurance or have no naturopathic coverage, we offer a 25% discount off of all provider service charges.

    OUT-OF-POCKET COSTS

It is important for you to understand your health insurance. If you would like to understand more about how health insurance works, see HERE. For some common insurance issues we have encountered, see HERE. There are some plans that are in-network but your plan is set to apply all costs of care to your deductible and only provide coverage once the deductible is met. This is most often seen with BRONZE or HSA plans. If you have one of these plans, you can estimate your out-of-pocket costs below:

While every visit is billed based on the individual care provided, below is an approximation of your base appointment cost. Please confirm these estimates before scheduling your first appointment, as costs below may not reflect recent increases. Also, please be aware these cost estimates do not factor in potential additional costs from labs, questionnaires, etc. Please make sure to discuss the full range of likely costs with someone at Northwest Integrative Medicine before scheduling your first appointment:

    New Patient Appointments (establishing care)

    New Visits: $220 – $433
    With 25% discount: $165 – $325

    Established Patient Appointments (follow-up visits)

    Established Patients: $179 – $354
    With 25% discounts. $134 – $265

Wellness Visits (preventive visits like well-child & annuals)

    Wellness Visits: New Patient $250 – $340
    With 25% discount: $188 – $254

    Labs

The out-of-pocket costs for labs are highly dependent on the lab and the test. Please discuss your options with your provider for ways to reduce the cost of your labs.

The above costs do not include procedure fees such as questionnaire screenings, in-office labs, office procedures, IV therapy, injections, osteopathic manipulations, bodywork, hydrotherapy, or the cost of any medications used in a visit or supplements prescribed following a visit. As a result, they are not a complete estimate of the likely cost for a visit.
