We are open to new patients (12 and up) and eagerly anticipating the chance to serve as your healthcare providers. By signing up below, you’ll let us know that you’re interested in receiving care from our clinic.

Please note, we use text messages extensively to help keep your care simple and connected, from appointment reminders to sending and receiving important paperwork. You’ll also need an email address to set up your secure patient portal. From time to time, we’ll send helpful updates by email so you always have the information you need.

To simplify billing, we require all patients to keep a credit card on file, with the exception of those covered by Medicaid.

We are now open to new patients.

Monday-Thursday: 9am-5:45pm
Friday: 9am-4:15pm
